The San Francisco Department of Public Health's Black Infant Health Program is sharing their updated referral form for providers and community-based organizations.
BIH Eligibility Requirements
- African-American Women, who identify as AA
- At least 18 years of age at enrollment
- No later than 30 weeks pregnant at enrollment
- San Francsco resident
The Black Infant Health Program aims to give Black babies a healthy start to life by support women's health and wellbeing during pregnancy.
